Intake Process

SilverLining SA (SLSA) makes sure that we are truly responsive to your needs.

As part of the process of working together and becoming your partner in long term support. We require you to be mindful of our step by step intake process. We require all documentation to be in place before providing services.

Step 1

Step 2

Participants eligibility can be assessed by our Support Coordinator after a brief discussion over the phone. We will let you know whether your needs can be supported by SLSA.

 

Eligibility Criteria Includes:

Be considered to have a disability under the Disability Act 2006 Be aged between 5 and 65 years of age living within the Adelaide region.

Step 3

To give a more detailed overview of our support and services offered, SLSA will set up a date and time to visit the participant and family in the comfort of their home or any place of their choice. This gives us an opportunity to meet the participant and build a positive rapport to enable us to gain a more detailed understanding of the participant, their support needs, their goals and the appropriateness of SLSA support. Service Agreement and Participant Intake Form will be completed during the process.

Step 4

In this process, SLSA already gained detailed understanding of what support and services the participant requires. SLSA will determine whether we have the capacity to provide the support and services requested. We are committed to provide high quality of service, rather than over-commit beyond capacity.

Step 5

Initial Support Plan including goal and expectations and Service quote will be emailed to the family for the participant / guardian to sign and return.

Step 6

Support and services will then commence if there are no other actions to be taken. We are more than happy to offer a no obligation visit beforehand or transitional visits if needed by the participant.

I can recommend Silverlining SA, speak to Glory and let her know what you need and your expectations and she will make sure her staff are trained and have back ups if someone can't make it and she has very high standards for her staff and welcomes feedback to make sure they are always improving. The staff also talk to each other and share information between each other to ensure continuity of care. When they started, Glory came out with her staff so she could see what was needed and was able to find the right staff.
